Where "site.com/primary/10-secondary/page" is the right address that works and brings up the right template, site map programs create addresses such as "site.com/10-primary/secondary/page" which doesn't. Maybe category data is mis-stored in my Db? Can sh404SEF solve this?

Maybe category data is mis-stored in my Db?Doubt it, more likely an Itemid issue. The sitemap extension is listing some links with different Itemid than your main link. This is a fairly common issue with many Joomla sites. If talking about com_content, yes, sh404SEF resolve that but at the cost of possibly changing all links on your site,which may or may not be acceptable.
