1. No, forms in AMP are complex and require custom coding usually. We support Joomla native contact form out of the box.
2. You should NOT create a sitemap for AMP pages. This is a mistake and it reduces the benefits of AMP.
3. it's rather the opposite. Your regular content (modules, template) are not used on the AMP version of a page (because they are not compatible with AMP and generally cannot be used). You can then publish some modules that you manually select onto the AMP page, do some styling to make them look good and use them.
Remember that:
- no regular CSS from your site can be used on the AMP page
- no external javascript can be used
