Hello,

Ok, I have the 404 page working, but the issue I'm having is it's displaying on the home page. I would like the 404 page to be a clean simple page. Here is how the 404 page is displaying now: http://www.xxxx.com/case_studies.html

Ah ha, I figured it out. I created a separate menu item and set the Item ID here: Itemid to use for 404 page

Hi

Yes, you got it. It's covered on this page of documentation.

Actually, you don't have to create a separate menu item, you can just use any existing one that has the appropriate display options (modules, etc).

Please note that though the home page configuration may not be the best, the 404 page should indeed be clean but show your usual menus for instance, so that user can find here way if the proposed similar urls don't fit their needs.
