How does sh404sef find the url's it generates. I keep deleting url's in the database & they keep reappearing shortly after.

sh404SEF doesn't generate URLs. Joomla and other extensions generate non-sef URLS (ie 'index.php?option=com_something...") and ask Joomla SEF or,when available, sh404SEF, to turn them into their SEF equivalent.
Whether you delete them or not, those URLS are present somewhere on your site and need to be SEF-ied before display.
