I have a two language site. When using the fields in Configuration > SEO > Page title like this:

Page title separator: | (default value)
Append to page title: text to append

I have mixed results. With the default language English, nothing is appended to the page title. For example, "Home", instead of "Home | text to append".

With the other language, Spanish, it appends "text to append" right after the page title, without the text separator or spaces. "Hometext to append"

What could cause this?

The Home page is an exception and we do not append the "Append to page title" value to it because many people wants to have a different "Append..." value for the home page. So the page title for the home page is simply what you enter in the "Home page meta" dialog.

Yes. As per the tooltip and the documentation on this page, the "Text to append" is used "raw", exactly as you enter it. If you want a space or a character between it and the rest of the title, insert it as you require.
The "Separator" is something else. From the doc:

Page title separator: this character is used only when sh404SEF builds the page title itself (basically for Joomla! com_content) and the title has multiple parts. In such instances, the character entered here will be used to separate the various parts. For instance, for a typical Joomla! article, the page title will be built as:
My article title | the article category
