I have something like this

Out of curiousity, and also because im creating articles programatically, once in a blue moon i see this "duplicate URL" being created, and I have no way to know the page loads wrongly because SEF404 chose a wrong URL until a user complains.

Im trying to prevent it, and to do that I would like to understand what causes Joomla to present TWO different URLs to SH404 and what i can do to prevent it.
